No Annual Maximum Dental Insurance
The Copay Plan is dental insurance with no annual maximum limitation, no network waiting periods and a large national network. It is a fully-insured PPO where employees pay copays and providers are reimbursed fee-for-service. The Copay Plan is an ideal alternative to DHMO plans, since The Copay Plan is true insurance and has no annual maximum.
Dental Insurance that Maximizes Benefits
Offers flexibility while providing low employee coinsurance and competitive premiums. With The PPO Plan (MAC), employees will enjoy significant savings by selecting from a network of PPO Providers. Employees may visit an non-network provider; however out-of-pockets costs will be higher.
Freedom of Choice Dental Insurance
Provides the freedom to select any dentist of your choice. You may select a dentist who is contracted with our extensive national PPO dentist network or you are free to visit a non-contracted provider. Coverage is comprehensive with either dentist selection. However, out-of-pocket costs will be higher when seeing a non-contracted provider.
